This entry form must be submitted by a parent or guardian and signifies parental consent and agreement with the rules of the contest. Only ONE ENTRY PER STUDENT may be submitted. An independent panel of NHSF board members will judge the artwork. All decisions are final. Entrant's poster, name, school, and grade may be shared publicly, which includes display or printing of the artwork and placement on the social media pages and website of the sponsoring organization for the purpose of program promotion. Winners will be recognized at New Haven Day and may also be announced in the school district newsletter.

Entries may be horizontal/landscape or portrait/vertical format. Only one entry may be submitted per student. Acceptable file formats are PDF, JPG, PNG, and GIF. We recommend a maximum uploaded file size of 11" X 17" for easier printing by the judges. Hard copy entries or original designs on paper will not be accepted.
